Population Health Management
Neighborhood Surveys-Residents in the two project
target areas are preparing to conduct a series of neighborhood surveys. The
interview instrument contains
questions formulated by SCCHP members, staff and resiedents, as well as
questions for the Louis Harris Local Population Health Improvement Survey.
The survey
measures residents' perceptions of a range of population health issues.
Residents and SCCHP staff issued the results of the initial survey, conducted
in 1999, to develop community improvement plans, and mobilize resources and
support from service providers and government agencies. The two neighborhoods
were galvanized by the improvement efforts that stemmed from these local plans.
Among their many population health improvement efforts, residents built neighborhood
associations, increased community safety, improved tenant/lanlord relations
and brought badly needed health and human services to their neighborhoods.
In addition to conducting neighborhood surveys, residents in the Corby/Olive
area recently completed a Neighborhood Assessment. The purpose of the assessment
is to reduce crime by improving the environmental design of the neighborhood.

Neighborhood Residents Campaign to Improve Their Neighborhood
Community mobilizer,
Michael Locklear, supported a half-dozen Corby-Olive area residents in a
well-constructed campaign to convince the City of Santa Rosa
Neighborhood Revitalization Program (NRP) to select their neighborhood
as one of only two new revitalization sites for fiscal year 2000-2001. Residents
strongly advocated for their community, faxing and sending letters of request
to city officials. Residents also organized special meetings and community
tours for City Council and NRP members. In the end, the NRP selected the
Corby Olive area in part because of the enthusiasm of the residents! The
Neighborhood Revitalization Program brings additional services to the neighborhood.
The effort primarily increases law enforcement, fire and code enforcement
and recreational activities. The Corby Olive Action Committee developed
a
vision statement to present to the NRP. The vision statement describes
what the residents hope their neighborhood will look like in one year. It is
the
roadmap for the resident and NRP neighborhood improvement activities.

Sonoma County Builds Alcohol (AOD) Prevention Policy Agenda
Together with the Department of Health Services Prevention staff, the Sonoma
County Community Health Partnership is convening a series of discussions to
build an alcohol and other drug (AOD) prevention agenda for Sonoma County.
Small groups of prevention providers and policy experts will discuss five broad
AOD prevention areas: environmental prevention, youth development, family services,
school-based prevention and criminal justice. Each group will review scientifically
proven prevention strategies to implement in Sonoma County.
A summary report will be generated to highlight gaps in service delivery and
identify opportunities to enhance provider capacity. The report will be used
to strengthen existing prevention programs and enhance opportunities for collaborative
alcohol and drug prevention work. The report findings will also guide a search
for additional prevention funding, the development of the Prevention Division's
Request for Proposals for AOD prevention funds, and the future direction for
the Health Partnership's AOD efforts.

Santa Rosa Neighborhood Surveys
What do the residents of the Southwest and West Steele Lane communities see
as their community assets? What challenges do their communities face? Sonoma
County Community Health Partnership members and residents will conduct neighborhood
surveys in the early summer to find the answers. The survey will provide background
information for community mobilization efforts as well as baseline data for
measuring quality of life in the two communities. Residents from the Southwest
and West Steele Lane neighborhoods will be trained and paid to conduct the
survey. The survey will be conducted in Spanish and English. Data should be
available in July.

Direct Services Implemented in Sonoma
The Sonoma County Community Health Partnership has made considerable progress
in direct service delivery. Several new projects were implemented in the past
six months that benefit residents in Southwest Santa Rosa and the West Steele
Lane areas. The new projects include expansion of health education classes
into community neighborhoods; a community mentoring and clean-up program engaging
youth in the West Steele Lane area; and a community newsletter that blossomed
into a full newspaper distributed to over 7,000 households.
The newspaper includes descriptions of Partnership services, health education
articles and notices of upcoming activities. It is also an excellent opportunity
to increase business involvement in Partnership efforts; the Partnership hopes
that eventually enough businesses will advertise in the newspaper to make it
a self-sustaining publication. This will also provide an opportunity to improve
the local economy by promoting local businesses. The newspaper is a project
in which residents take full pride, as it is truly their product.
